Schlamp Family Medical Clinic Information
Appointments may be made by calling the office during regular office hours. In order to allow adequate time, you will be asked the nature of your visit. If the reason for your visit is urgent in nature and requires a same day visit, we will do our best to schedule you on that day. Because of emergencies, we sometimes run behind schedule. When this happens, we will notify you and do our best to assist you as soon as possible.
When an emergency requires immediate attention, go directly to the hospital Emergency Room. If it is during regular clinic hours, please call the office and you will be seen immediately. After regular clinic hours, call the office and the answering service will contact Dr. Schlamp or the physician on call concerning the emergency.
Call the office at 337-527-6385 or email us. We do our best to answer all inquiries as quickly and efficiently as possible.
Should you find it necessary to change or cancel an appointment, please notify our office within 24-hours. This courtesy will allow someone else to use that available time.
If you have seen another physician, it is helpful to have his/her previous office records. Consent for release of information must be filled out and signed,
