This is the most innovative addition to our website as of yet. The portal allows you to securely LOG ON to request appointments, review your personal health information, complete and update your medical forms, keep us updated on your contact information, send messages to our practice, review and pay outstanding billing statements, and MUCH more!
Our goal is to provide the best care possible to our patients in the most efficient way possible. The addition of the Athena Health PATIENT Portal to our website was made in order to help you to communicate with us in a quick, secure, and confidential manner!
To sign up, view the “Athena Health Patient Portal” icon in the top right corner of the SCHLAMPMD.com website.
You will then be redirected to our secure online patient portal. If you HAVE signed up online for an account with us already, then you will enter your PIN, Home/Primary Phone number, and Date of Birth into the fields and then click Sign In.
If you are a NEW patient, or EXISTING patient who has never used the online portal, click “Create an Account” under the question “Are you new to the patient portal?”
It’s as simple as that! We hope that you will take advantage of the resources that we provide to make your experience with Schlamp Family Medical Clinic as convenient as possible.
