Medicare and Humana Gold Chronic Care Management services are now available! Chronic conditions are ongoing medical problems that must be managed effectively in a partnership between the health care team and you, to maintain your best possible health.

Paid by Medicare, it is a monthly medical assessment in addition to your regular office visits. The assessment is conducted over the phone, where we coordinate your medical care with other doctors, hospitals, diagnostic facilities, and pharmacies.
