Fees and PaymentPayments of co-pays, deductibles, or any fees not covered by your insurance company are expected at the time of service. We accept cash, check, care credit, and most major credit cards. While we expect you to keep our account current, if you are experiencing financial difficulties, please let the office manager know so that we may take it under consideration. We value you as a patient.

InsuranceWe will file all claims to our participating insurance groups. We encourage you to become familiar with your insurance plan. Please, know where your insurance company is located, when your policy became effective, and an employer contact person.

Appointments and Walk-insAppointments may be made by calling the office during regular office hours, Monday - Wednesday 8AM-5PM and Thursday & Friday 7:30AM-5PM. You may request an appointment through the patient website, by filling out our appointment request form, text message to 1.833.682.7682, or email.
If you require a same day visit, we will do our best to schedule you on that day. Because of emergencies, we sometimes run behind schedule. When this happens, we will notify you and do our best to assist you as soon as possible. |
EmergenciesWhen an emergency requires immediate attention, go directly to the hospital Emergency Room. If it is during regular clinic hours, please call the office and you will be seen immediately. After regular clinic hours, call the office and the answering service will contact Dr. Schlamp or the physician on call concerning the emergency.

Prescription RefillsWe encourage you to refill all medications during a regular office visit. If this is not possible, please contact your pharmacy and they will notify us via fax. Please allow two business days for all refills.
