Schedule your appointment with our FNP, Angie Ewalt! 
Angie Ewalt Dukes, a Sulphur native and US Navy Veteran, is a Board-certified Family Nurse Practitioner licensed by the Louisiana State Board of Nursing to provide health-care services.
She began her nursing career as a CNA at Holly Hill Nursing Home in 1993.
​From there, she joined the US Navy where she served as a dental technician for six years. After her honorable discharge, she returned to the Lake area and enrolled at McNeese State University.
She earned her Associate's RN degree in 2002 and attained her BSN in 2012. She completed her Master's in Science-Nursing in 2020 from Northwestern State University.

The 20 years of nursing experience she brings with her into primary care include emergency medicine and inpatient cardiology. She completed clinical rotations in pediatric, adult, geriatrics, pulmonology, cardiology, and urgent care settings. 

It is estimated that over 80,000 people die from causes associated to alcohol each year, 5,000 of those being under the age of 21. According to the National Highway Traffice Safety Administration, over 11,000 people died in alcohol impaired driving accidents in 2020. 
Over-indulgence in alcohol can cause liver disease/cancer, high bp, heart diseases, stroke, colon cancer, a weakened immune system, dementia, depression, anxiety and alcoholism/alcohol dependency.

Chronic Care

Chronic problems must be managed by your PCP, & a healthcare team, to maintain the best possible health. It is a monthly medical assessment done over the phone.
Centers for Medicare & Medicaid Services recognizes Chronic Care as a crucial part of primary care.


Stress can take a monumentous toll on your mind and body. It is very important to learn how to cope with stress in a HEALTHY WAY.

There are three major steps in defeating diabetes.

Type 1 diabetes occurs at every age and of every race, shape & size. It can be managed by living and practicing a healthy lifestyle. 
Type 2 diabetes is the most common form. Some people may be able to manage this form of diabetes with a healthy lifestyle, while others will need insulin. A pertinent part of managing type one diabetes is focusing on a healthy diet. 

Remote Patient Monitoring

We have access to blood pressure cuffs you may use at home.
​This bp cuff will send our office your results through an app.

Testicular Cancer

Testicular cancer accounts for only 1% of cancers in men. It is the most commonly diagnosed cancer in men aged 15-40. 
Men should be aware of the signs and symptoms of testicular cancer. 
1. The most common symptom is swelling or discomfort in the scrotum. 
2. Lump in either testicle (may be painless)
3. Ache in the groin or lower abdomen
4. Pain or discomfort in either testicle
The key to early detection is self-examination


Treatments may include surgery, chemo, radiation therapy and stem cell transplant.

Telemedicine

Telemedicine is the exchange of medical information through electronic communications such as, laptop, cell phone, or tablet.
This is something you can do from the comfort of your own home and is very easy to do. 

You must fill out a telemedicine consent form every year.
